Value of diurnal variation in ventricular late potential on dynamic electrocardiogram in risk assessment of fatal arrhythmia after myocardial infarction

Abstract:Objective To explore the value of diurnal variation of ventricular late potential(VLP)in dynamic electrocardiography(ECG)for assessment of the risk of fatal arrhythmias after myocardial infarction(MI).Methods A total of 206 MI patients admitted to Department of Cardiovascular Diseases of Tengzhou Central People's Hospital from May 2021 to May 2024 were recruited and served as MI group.Based on fatal arrhythmias occurred or not,these patients were divided into an arrhythmia subgroup(54 cases)and a non-arrhythmia subgroup(152 cases).Another 76 non-MI patients admitted during the same period were also subjected as the control group.ECG workstation late potential analysis system was used to collect total time required for ventricular depolarization(TQRS)and record VLP positivity.The changes in diurnal TQRS and VLP positivity were compared between the MI group and the control group,and between the arrhythmia subgroup and the non-arrhythmia subgroup.Results Both the MI group and the control group had significantly higher nighttime TQRS value than daytime value(P<0.01),and the MI group had obviously higher daytime and nighttime TQRS values than the control group(P<0.01).The positive rate of VLP was significantly higher in the MI group than the control group(24.8%vs 3.9%,P<0.01).The nighttime TQRS value of both the arrhythmia subgroup and the non-arrhythmia subgroup was significantly higher than the daytime value(P<0.01);The daytime and nighttime TQRS values were significantly higher in the arrhythmia subgroup than the non-arrhythmia subgroup(P<0.01).The positive rate of VLP was significantly higher in the arrhythmia subgroup than the non-arrhythmia subgroup(48.1%vs 16.4%,P<0.01).Pearson correlation analysis showed that the risk of fatal arrhythmia was positively correlated with the mean diurnal TQRS value and VLP positivity(r=0.541,r=0.676,P<0.01).Conclusions VLP is of important value in assessing the risk of fatal arrhythmia after MI,and the risk of fatal arrhythmia is significantly correlated with diurnal TQRS and VLP positivity,which is worthy of clinical reference.
