Expert Consensus on Antithrombotic Therapy for Elderly Patients with Chronic Coronary Syndrome 2025
Abstract:Chronic coronary syndrome (CCS) refers to a series of clinical manifestations or syndromes caused by structural and/or functional changes related to coronary artery and/or chronic microcirculation diseases, excluding acute coronary syndromes (ACS). It includes stable angina, ischemic cardiomyopathy, stable conditions after ACS or coronary revascularization, asymptomatic myocardial ischemia, coronary microvascular disease, coronary spasm, and asymptomatic coronary heart disease (CHD) [1-3]. Elderly CCS patients generally exhibit physiological function decline, prolonged disease course, multiple comorbidities, polypharmacy, cognitive impairment, and poor adherence, which are not only important causes of mortality but also pose significant challenges for antithrombotic therapy [4]. Currently, there is a lack of targeted large-scale randomized controlled trials and guidelines for antithrombotic therapy in elderly CCS patients both domestically and internationally. To address the practical needs of antithrombotic therapy for elderly CCS patients in clinical settings, the Thrombosis Prevention Branch of the Chinese Society of Geriatric Medicine and the Editorial Committee of the Chinese Journal of Geriatric Cardiology and Cerebrovascular Disease have organized domestic experts to jointly develop the 'Expert Consensus on Antithrombotic Therapy for Elderly Patients with Chronic Coronary Syndrome 2025', aiming to provide a basis for standardized antithrombotic therapy for elderly CCS patients at all levels of medical and management institutions. In this consensus, elderly patients are defined as those aged ≥80 years.
