Study on the Expression Levels of Poly-ADP-Ribosomal Polymerase 1 and Nuclear Factor E2-Related Factor 2 in Elderly Patients with Arrhythmia
Abstract:Objective To investigate the differences in serum levels of poly-ADP-ribosomal polymerase 1 (PARP1) and nuclear factor erythroid 2-related factor 2 (Nrf2) in elderly patients with arrhythmia with different left ventricular ejection fractions (LVEF). Methods A retrospective study was conducted on 240 elderly patients with arrhythmia diagnosed at the Department of Cardiology, The Third Hospital Affiliated to the School of Medicine, Xi'an Jiaotong University, from January 2022 to January 2024. The patients were divided into three groups based on LVEF: high-ejection fraction group (n=88), middle-ejection fraction group (n=96), and low-ejection fraction group (n=56). A total of 60 healthy individuals undergoing physical examination at the same hospital during the same period were selected as the control group. General data and serum levels of PARP1 and Nrf2 were compared between the study group and the control group. The types of arrhythmia and serum levels of PARP1 and Nrf2 in patients with different LVEF were compared, and the correlation between serum PARP1 and Nrf2 levels and LVEF in elderly patients with arrhythmia was analyzed. Results The LVEF level in the study group was significantly lower than that in the control group [(36.31±10.57)% vs (61.25±18.57)%, P=0.000]. There were significant differences in the types of arrhythmia among the high-ejection fraction group, middle-ejection fraction group, and low-ejection fraction group (P<0.01), with the low-ejection fraction group mainly presenting with ventricular tachycardia. Serum PARP1 levels in the study group were significantly higher than those in the control group [(9.53±3.18) μg/L vs (6.74±1.43) μg/L, P=0.000], while serum Nrf2 levels were significantly lower [(0.94±0.37) μg/L vs (1.85±0.31) μg/L, P=0.000]. Serum PARP1 levels in the low-ejection fraction group were significantly higher than those in the middle-ejection fraction group and high-ejection fraction group, and serum Nrf2 levels were significantly lower than those in the middle-ejection fraction group and high-ejection fraction group (P<0.05). Serum PARP1 levels in the middle-ejection fraction group were significantly higher than those in the high-ejection fraction group, and serum Nrf2 levels were significantly lower (P<0.05). There was a significant positive correlation between serum PARP1 levels and LVEF (r=0.564, P=0.000), and a significant negative correlation between serum Nrf2 levels and LVEF (r=-0.486, P=0.000). Conclusion Elderly patients with low LVEF are prone to develop severe ventricular tachycardia, which is positively correlated with serum PARP1 and negatively correlated with serum Nrf2.
