Abstract:Objective To make a cross-sectional study on the changes in cognitive neural networks at different stages of Alzheimer's disease(AD)progression by comparing the characteristics of resting-state electroencephalogram(EEG)microstate parameters in order to provide electrophysio-logical basis for the pathogenesis and early diagnosis of AD.Methods A total of 84 patients with cognitive impairment admitted to our department from September 2018 to June 2021 were en-rolled and divided into AD group and mild cognitive impairment(MCI)group according to the di-agnostic criteria for AD by the National Institute on Aging, with 42 cases in each group.Another 42 healthy individuals who taking physical examination during the same period were subjected and served as control group. The EEG data of the three groups were collected and preprocessed, and then microstate analysis was conducted by means of one plugin in EEGlab.Four types of microsta-te prototypes were set up,and the temporal parameters of resting-state brain network microstate were extracted and compared among the three groups. Results The coverage rate of class D mi-crostates was significantly higher in the AD group than that the MCI group[(0.27±0.10)%vs(0.16±0.11)%,P<0.05].The incidence of class D microstates was higher in the AD group than the control group and MCI group[(2.93±0.79)%vs(2.56±0.77)%and(1.78±0.67)%,P<0.01].The rate of microstate transition from classes B,C and D to class A was higher in the AD group than the MCI group[(0.3617±0.1444)%vs(0.2115±0.0908)%,(0.3363±0.1386)%vs(0.2262±0.1086)%,(0.3506±0.1198)%vs(0.1959±0.1161)%,P<0.05].The transition rate from class B microstate to class C microstate was higher in MCI group than that in AD group[(0.5058±0.2038)%vs(0.3169±0.1410)%,P<0.05].The transition rate from classes B and D microstate to class A microstate was higher in the control group than the MCI group[(0.3262± 0.1670)%vs(0.2115±0.0908)%,(0.3287±0.1142)%vs(0.1959±0.1161)%,P<0.05].Con-clusion The increased coverage and incidence of class D microstate may be the parameters that distinguish AD from MCI.The change of microstate transition rate of classes A and C in MCI stage may be a specific parameter that distinguishes MCI from normal subjects.
