Characteristics and diagnosis and treatment of heart failure in hospitalized patients in Henan Province

Abstract：Objective To analyze the clinical characteristics and diagnosis and treatment status of heart failure(HF)patients in Henan Province.Methods A cross-sectional survey was conducted on 20 620 HF patients hospitalized in 45 hospitals of the Heart Failure Center Alliance of Henan Province from January 2018 to December 2021(the data from China Cardiovascular Association Database-Heart Failure Center).Their age,sex,body mass index,education level,history of smok-ing,history of alcohol consumption,blood pressure,heart rate,etiology,comorbidities,symptom-atic features,signs,left ventricular ejection fraction,blood levels of potassium,sodium,hemoglo-bin and creatinine,medications,and device therapy were collected from all the patients.Results The HF patients in Henan Province were at an average age of 69.0±13.0 years,and 56.4％of them were males.The main causes for HF were coronary heart disease(70.9％),dilated cardiomy-opathy(8.4％)and valvular heart disease(7.6％).The most common comorbidity was coronary artery disease(76.5％),followed by hypertension(56.9％)and anemia(44.6％).Their mean length of hospital stay was 10(7,14)d.There were 5073 patients(27.4％)with HFrEF,4940 pa-tients(26.7％)with HFmrEF,and 8519 patients(46.0％)with HFpEF.Compared with the data of Report of Consortium of China Heart Failure Center(2022),the distribution of HF types was also HFpEF predominately;the main etiology was also coronary heart disease;the proportion of device treatments for cardiac resynchronization therapy(such as CRT-D,CRT-P,and ICD)was lower than the national level,but the difference was not significant(CRT-D:0.5％vs 1.8％,CRT-P:0.2％vs 0.6％,ICD:0.4％vs 1.8％,P＞0.05).Conclusion In Henan province,the main type of HF is HFpEF,main etiology is coronary heart disease,and the Proportion of device therapy is quite low.So,there is still a gap between guideline recommendations and guideline-based guidance on drug application and device therapy for HF patients.
