Predictive value of systemic immune-inflammatory index for MACE in patients with coronary heart disease complicated with heart failure

Abstract：Objective To investigate the relationship between systemic immune-inflammatory in-dex(SII)and MACE in patients with CHD and HF.Methods A total of 354 patients with CHD combined with HF hospitalized in North Jiangsu People's Hospital from June 2018 to June 2022 were enrolled and followed up until June 2023.According to the occurrence of MACE,the patients were divided into non-MACE group(180 cases)and MACE group(174 cases).The patients with SII were divided into three groups according to tertiles:low interquartile group(SII＜633,117 cases),middle interquartile group(633≤SII＜1354,119 cases)and high interquartile group(SII≥1354,118 cases).For these CHD and HF patients,ROC curve analysis was used to analyze the di-agnostic value of SII for MACE,and Kaplan-Meier curve and Cox proportional risk analysis were employed for the relationship between SII and the risk of MACE.Results Age,creatinine,uric acid,neutrophil count,NLR,SII,NT-proBNP and mitral regurgitation were higher in the MACE group than the non-MACE group,platelet counts were lower in the MACE group than in the non-MACE group(P＜0.05,P＜0.01).When SII was used to predict MACE,the AUC value was 0.81(95％CI:0.76-0.86).Kaplan-Meier survival curve analysis showed that the high score group had lower cumulative survival rate than the middle score group and the low score group(P log rank＜0.01).Multivariate Cox regression analysis indicated that high(HR=5.15,95％CI:2.99-8.86,P=0.001)and middle scores(HR=9.22,95％CI:5.43-15.64,P=0.001)were risk factors for MACE.Conclusion Higher SII is an independent risk factor for MACE in patients with CHD and HF,so it can be used to identify high-risk population with poor prognosis earlier.
