Accuracy of Doppler ultrasonography in diagnosis of renal artery stenosis in elderly patients
Abstract:Objective To assess the accuracy of color Doppler ultrasonography in diagnosis of atherosclerotic renal artery stenosis (ARAS) according to its receiver operating characteristic (ROC) curve.Methods The peak systolic velocity (PSV) was measured and the renal artery hemodynamic parameters were recorded in 259 elderly patients by color Doppler ultrasonography.The RAR and RIR were calculated.The ROC curve was plotted with renal arteriography as its golden standard.The sensitivity,specificity and accuracy of renal artery hemodynamic parameters were compared.Results The threshold of PSV,RAR,RIR for diagnosing moderate ARAS (50%-69%) and severe ARAS (70%-99%) was 158 cm/s and 248 cm/s,2.0 and 4.2,5.05 and 10.67 respectively.The sensitivity and specificity of 158 cm/s≤PSV<248 cm/s,2.0≤RAR<4.2,5.05≤RIR<10.67 for diagnosing moderate ARAS were 91.2% and 89.7%,92.9% and 86.2%,95.5% and 86.2% respectively.The sensitivity and specificity of PSV≥248 cm/s,RAR≥4.2,RIR≥10.67 for diagnosing severe ARAS w ere 92.0% and 85.0%,90.0% and 80.0%,93.0% and 85.0% respectively.Conclusion PSV,RAR,RIR are the renal artery hemodynamic parameters of color Doppler ultrasonography for accurate diagnosis of ARAS in elderly patients.
