Comparison of four kinds of stroke risk score in the diagnosis of posterior circulation ischemia isolated vertigo
Abstract:Objective To evaluate the value of four stroke risk scores used in the diagnosis of posterior circulation ischemia isolated vertigo.Methods 409 patients with the only complaint of dizziness/vertigo were scored according to the Framingham stroke profile (FSP),age,blood pressure,clinicalfeatures,durationofsymptoms,diabetes (ABCD2),Essen stroke risk score (ESRS) and ischemic cardiovascular disease (ICVD) within 24 h after admission.The patients were divided into posterior circulation ischemia isolated vertigo group (group A,n=77) and non-posterior circulation ischemia isolated vertigo group (group B,n=332) according to their clinical characteristics and auxiliary examination findings.The role of the 4 scales in diagnosis of posterior circulation ischemic isolated vertigo was assessed according to the ROC curve.Results The FSP,ABCD2,ESRS and ICVD scores were higher in goup A than in group B (A group:males 20、females 19;3;3;males 9、females 9;B group:males 15、females 14;2;1;males 6、females 6,P< 0.01).The area under the ROC curve was 75.8%,75.9%,77.7%,75.6% respectively for the 4 scales.The score of the 4 scales in predicting posterior circulation ischemia isolated vertigo was 18,3,3 and 6 respectively with an accuracy of 72.61%,69.68 %,74.82 % and 68.46 %.Conclusion The 4 scales play an important role in diagnosis of posterior circulation ischemia isolated vertigo.
