Risk factors for left atrial thrombosis in elderly nonvalvular AF patients
Abstract:Objective To study the risk factors for left atrial thrombosis in elderly nonvalvular atrial fibrillation (NVAF) patients.Methods One hundred and sixteen elderly NVAF patients with transesophageal echocardiography-confirmed LA/LAA thrombosis served as a thrombus group and 116 NVAF patients without LA/LAA thrombosis served as a control group.The risk factors for LA/LAA thrombosis in two groups were analyzed by multivariate logistic regression analysis.Results The incidence of hypertension and ischemic stroke was significantly higher in thrombus group than in control group (65.52% vs 51.72 %,13.79% vs 5.17%,P<0.05).The prevalence of congestive heart failure was higher,the left atrial diameter (LAD) and left ventricular diastolic diameter were longer,the serum NT-proBNP level and CHA2DS2-VASc score were higher,the persistent/permanent atrial fibrillation (AF) ratio was greater in thrombus group than in control group (P<0.01).Multivariate logistic regression analysis showed that persistent/permanent AF ratio,serum NT-proBNP level,CHA2DS2-VASc score and LAD were the independent risk factors for left atrial thrombosis in elderly NVAF patients (OR =5.721,95 % CI:2.069-15.820,P =0.001;OR=1.001,95%CI:1.000-1.002,P=0.002;OR=2.021,95%CI:1.316-3.103,P=0.001;OR=1.104,95%CI:1.002-1.216,P=0.046).Conclusion Persistent/permanent AF ratio,LAD,serum NT-proBNP level and CHA2DS2-VASc score are the risk factors for left atrial thrombosis in elderly NVAF patients.
