Risk factors for symptomatic hemorrhagic transformationin and clinical outcome in severe AIS patients after thrombolytic therapy
Abstract:Objective To study the risk factors for symptomatic hemorrhagic transformation (SICH) in severe acute ischemic stroke (AIS) patients (NIHSS score >25) after thrombolytic therapy and their long-term outcome.Methods Eighty severe AIS patients for thrombolytic therapy were divided into SICH group (n=13) and non-SICH group (n=67).The risk factors for SICH,the outcome and end point mortality in two groups were analyzed after 90 days of thrombolytic therapy.Results The blood glucose level and SBP after thrombolysis were higher,the thrombolytic time was >3 h,the incidence of AF and anterior circulation infarction was higher in SICH group than in non-SICH group (P<0.01).Logistic regression analysis showed that the blood glucose level on admission,SBP after thrombolysis,AF,thrombolytic time >3 h,and anterior circulation infarction were the risk factors for SICH in severe AIS patients after thrombolytic therapy(OR=2.423,95%CI:1.342-7.450,P=0.023;OR=1.159,95%CI:1.028-2.240,P=0.029;OR=3.715,95%CI:1.649-8.370,P=0.002;OR=4.604,95%CI:1.952-10.860,P=0.001;OR 4.779,95%CI:1.903-12.004,P=0.001).In SICH group,3 (23.1%) had a good outcome,10 (76.9%) had a poor outcome and 3 (23.1%) died.In non-SICH group,23 (34.3%)had a good outcome,44 (65.7%) had a poor outcome and 9 (13.4%) died.Conclusion The outcome is poor in severe AIS patients with SICH after thrombolytic therapy.
