Value of modified ABCD3 score in assessment of stroke recurrence after transient ischemic attack
Abstract:Objective To study the value of modified ABCD3 score in assessment of stroke recurrence after transient ischemic attack (TIA).Methods One hundred and twenty-seven TIA patients,scored according to ABCD2,ABCD3 and modified ABCD3,were divided into mild risk group,moderate risk group and severe risk group.Recurrence of ischemic stroke was recorded for 7 days after TIA.The value of ABCD2,ABCD3 and modified ABCD3 3 scores in assessment of stroke recurrence after TIA was evaluated according to the ROC curve.Results Of the 127 TIA patients,ischemic stroke reoccurred in 26 (20.5%).The ratio of recurrent ischemic stroke was higher in patients whose TIA frequency was >3 than in those whose TIA frequency was 1 or 2-3 (37.9% vs 13.0%,37.9% vs 17.3%,P<0.05).Significant difference was found in risk stratification between ABCD2,ABCD3 and modified ABCD3 scores (P =0.000),between ABCD2 and ABCD3 scores (P=0.004),between modified ABCD3 and ABCD2 scores (P=0.000).The recurrence of ischemic stroke was higher in high risk group according to the modified ABCD3 score than in those according to the ABCD2 and ABCD3 scores (36.3% vs 31.2%,36.3% vs 28.5%,P<0.05).The area under the ROC curve for modified ABCD3 score was 0.713 (95%CI:0.604-0.823,P=0.001).Conclusion Modified ABCD3 score is better than ABCD2 and ABCD3 scores in predicting the recurrence of ischemic stroke in high risk patients,and can thus predict the risk of stroke recurrence in early TIA patients.
