Change trends of clinical characteristics, therapies and in hospital events in elderly patients with acute coronary syndrome
Abstract:Objective To study whether clinical characteristics,therapies and in-hospital events of elderly patients with acute coronary syndrome (ACS) changed over time.Methods This study enrolled 944 ACS patients aged ≥ 60 years hospitalized in Department of Cardiology,Peking University First Hospital,including 628 patients aged 60-74,316 patients aged 75-89,of them,162 patients in 2000,293 patients in 2007,489 patients in 2014,respectively.Temporal trends of clinical features,guideline-recommended therapies and in-hospital events were analyzed.Results From 2000 to 2014,patients were older,percentage of comorbidities such as hypertension,diabetes and hyperlipedemia increased (for hypertension,increased from 66.0% to 75.5%,for DM,from 21.6% to 43.1%,for hyperlipedemia,from 12.3% to 44.0%).The proportion of patients who underwent previous CAG and PCI increased (for CAG,from 18.5% to 40.9%,for PCI,from 8.0% to 33.3%).Patients with symptomatic heart failure and infection at admission decreased.AMI patients took up a lesser proportion in 2007 and 2014,compared with those in 2000.UA patients increased markedly in 2014.Use of guideline-recommended drugs increased.PCI use increased from 34.6% in 2000 to 70.6% in 2014 (P<0.01).Meanwhile,in-hospital mortality decreased from 11.1% in 2000 to 1.6% in 2014,and in-hospital MACCE from 21.6% to 8.4% (P<0.01).Conclusion Clinical characteristics changed,use of guideline-recommended drugs and PCI increased over time,and in-hospital outcomes improved in elderly patients with ACS.
