Risk factors for progressive early ischemic stroke in patients with internal watershed infarction due to severe middle cerebral artery stenosis
Abstract:Objective To study the risk factors for progressive early ischemic stroke in patients with internal watershed infarction (IWI) due to severe middle cerebral artery (MCA) stenosis.Methods Eighty-four patients with IWI due to severe MCA stenosis admitted to our hospital from January 2012 to May 2016 were divided into progressive IWI group (n=47) and stable IWI group (n=37).Their clinical data,ischemic stroke patterns and collateral circulation were compared.Results The incidence of diabetes mellitus and blood pressure were significantly higher while the regional leptomeningeal score (rLMC) score was significantly lower in progressive IWI group than in stable IWI group (48.9% vs 27.0%,78.7% vs 43.2%,5.30±2.21 vs 8.03±4.25,P<0.05,P<0.01).No significant difference was found in IWI,IWI+EWI,IWI+PAI and IWI+PI between the two groups (P>0.05).However,the incidence of ischemic stroke was significantly higher progressive IWI group than in stable IWI group (25.5% vs 5.4%,P<0.05).Multivariate logistic regression analysis showed that rLMC was an independent risk factor for progressiive ischemic stroke in patients with IWI due to severe MCA stenosis (OR=1.264,95%CI:1.052-1.519,P =0.012).Conclusion rLMC is an independent risk factor for progressive ischemic stroke in patients with IWI due to severe MCA stenosis.
