Factors associated with out of hospital cardiac arrest in elderly patients and their outcome
Abstract:Objective To study the factors associated with the outcome in elderly patients with out of hospital cardiac arrest (OHCA) by analyzing its epidemiological characteristics.Methods Six hundred and twenty-five OHCA patients admitted to our hospital,Shaoxin Emergency Center,and Ningbo Emergency Center from January 2012 to October 2016 were divided into old-aged group (n =326) and middle-and young-aged group (n =299).Their clinical data,epidemiological characteristics and outcome of OHCA were analyzed by logistic regression analysis.Results The first incidence of OHCA at home,the ratio of initial rhythm manifestaed as non-shockable rhythm,the ratio of OHCA manifested as cardiac pathogenic cause and witnessed by bystanders were significantly higher in old-aged group than in middle-and young-aged group (79.1% vs 47.5%,89.9% vs 84.6%,52.5% vs 44.5%,77.6% vs 70.6%,P<0.05,P<0.01).The ROSC on the spot,on the way and in emergency department,the survival rate in hospital and after discharge were significantly lower in old-aged group than in middle-and young-aged group (P<0.05,P<0.01).Shockable rhythms as the initial rhythm,emergency response time <10 min,bystander CPR and prehospital defibrilation were the independent risk factors for ROSC in elderly OHCA patients.Conclusion OHCA is characterized by its onset site,initial rhythm,pathogenic cause witnessed by bystanders in elderly patients.The outcome in elderly patients is rather poor than middle-and young-aged patients.Emergency measures can improve their ROSC rate.
