Acute lower respiratory infection increases the incidence of atrial fibrillation in very old hypertensive patients
Abstract:Objective To study the incidence of atrial fibrillation (AF) and its relative risk factors in very old hypertensive patients with acute lower respiratory tract infection.Methods Three hundred and forty-one >80 years old hypertensive patients admitted to our hospital from January 2011 to December 2013 were divided into infection group (n=192) and infection-free group (n=149).The incidence of AF in the patients on days 7,14,30 and 90 after admission was analyzed.The patients were further divided into new-onset AF group (n=11) and new-onset AF-free group (n=330).Results The incidence of AF was significantly higher in infection group than in infection-free group during the 90 days follow-up period (5.73% vs 0%,P=0.003).The incidence of acute lower respiratory tract infection and its etiology were significantly different between newonset AF group and new-onset AF-free group (P<0.05).Cox regression analysis showed that heart rate,infection type and LVEF on admission were the independent risk factors for new-onset AF during the 90 days follow-up period (HR =1.048,95 % CI:1.001-1.098,P =0.048;HR =113.815,95 %CI:3.430-3777.011,P=0.008;HR=1.640,95%CI:1.058-2.541,P=0.027).Condusion Acute lower respiratory tract infection significantly increases the incidence of new-onset AF in very old hypertensive patients,its effective prevention and treatment can thus improve their poor outcome.
