Influencing factors of poor outcome in elderly AIS patients
Abstract:Objective To study the influencing factors and clinical outcome of hemorrhagic transformation (HT) in elderly with acute ischemic stroke (AIS) patients after intravenous thrombolysis.Methods One hundred and two elderly AIS patients were divided into non-HT group (n=81) and HT group (n=21) according to their clinical symptoms and CT findings.Their clinical outcome was assessed according to the mRS.Good outcome was defined when the mRS score was 0-2 and poor outcome was defined when the mRS score was 3-6.The patients were scored according to the NIHSS before and 24 h after thrombolysis.Independent risk factors for HT and poor clinical outcome in elderly AIS patients were analyzed by multivariate logistic regression analysis.Results The NIHSS score for poor outcome was significantly higher in HT group than in nonHT group before and 24 h after thrombolysis (P<0.05,P<0.01).Multivariate logistic regression analysis showed that NIHSS score before thrombolysis (OR=1.253,95%CI:1.162 1.335,P=0.013) and 24 h NIHSS score after thrombolysis (OR=1.427,95 % CI:1.374 1.526,P=0.006) were the independent risk factors of HT.Age and NIHSS score before and 24 h after thrombolysis were the independent risk factors for poor outcome in elderly AIS patients (OR=0.956.95%CI:0.863-1.125,P=0.042;OR=1.072,95 % CI:0.984-1.186,P=0.038;OR=1.516,95%CI:1.383-1.762,P=0.012).Conclusion Age and NIHSS score before and 24 h after thrombolysis are the independent risk factors for poor outcome in elderly AIS patients.
