Association between carotid artery assessment before operation and neurological complications after OPCAB
Abstract:Objective To study the association between carotid artery assessment before operation and neurological complications (NC) after off‐pump coronary artery bypass (OPCAB) .Methods Five hundred and fifty‐six patients who underwent OPCAB from July 2010 to July 2012 in our hospital were included in this study .Their general condition ,past medical history ,head CT scan‐ning and carotid ultrasonography data were recorded before operation .Their nerve function was assessed before and 7 days after operation in order to judge whether NC ocurred .The patients were divided into NC group (n=118)and control group (n=438) .The association between carot‐id artery stenosis and risk factors for postoperative NC was analyzed .Results The total incidence of NC was 21 .2% .The incidence of ischemic stroke ,anoxic encephalopathy ,delirium ,postopera‐tive cognition impairment ,and anxiety/depression was 0 .7% ,2 .9% ,0 .7% ,10 .6% and 4 .9% re‐spectively .The incidence of ischemic stroke and carotid artery stenosis was significantly higher in NC group than in control group (P<0 .05) .Multivariate logistic regression analysis showed that the incidence of postoperative NC increased with the severity of carotid artery stenosis (OR=2.23 ,95% CI:1 .08 -4 .61 ,P= 0 .030) .Conclusion The incidence of NC following OPCAB in‐creases with the severity of carotid artery stenosis .
