Value of arterial stiffness in predicting adverse cardiac events
Abstract:Objective To study the value of arterial stiffness (AS) index in predicting adverse car-diac events(ACE ) .Methods One hundred and ninety-eight coronary heart disease (CHD ) pa-tients were divided into increased AS group (n= 87) and control group (n= 111) according to whether their central pulse pressure (CPP) was >45 mm Hg .The patients were followed up for 52 ± 12 months ,during which their ACE were compared .The independent factors for ACE were analyzed by Cox regression analysis .Results The blood pressure was significantly higher in in-creased AS group than in control group (P<0 .01) .During the follow-up period ,end point events occurred in 42 patients (major ACE in 20 and minor ACE in 22) .The incidence of major ACE and end point events was significantly higher in increased AS group than in control group (18 .4% vs 3 .6% ,P=0 .001;33 .3% vs 11 .7% ,P=0 .001) .Cox regression analysis showed that CPP > 45 mm Hg (RR= 3 .86 ,95% CI:1.89 -7 .87 ,P< 0 .001) ,common carotid atherosclerotic plaques (RR=2 .17 ,95% CI:1 .38-3.41 ,P=0 .001) ,coronary artery lesion (RR=1 .70 ,95% CI:1 .24 -2 .34 ,P=0 .001) and history of cerebral vascular disease (RR=3 .26 ,95% CI:1 .41 -7 .52 ,P=0.007) were the influence factors for the prognosis of cardiovascular diseases .Conclusion CPP>45 mm Hg and common carotid atherosclerotic plaques are the predictors of ACE .Noninvasive AS indexes are of great value for assessing the prognosis of cardiovascular diseases and guiding their prevention .
