Abstract:This study presents a rare case of a bilateral second mandibular molar with eight cusps, with pulp infection observed in the left mandibular second molar secondary to dental caries. To investigate the clinical characteristics and treatment approaches for malformed teeth, comprehensive assessments were conducted using intraoral examinations, dental model measurements, radiographic imaging, and digital oral scanning.Following root canal therapy, the buccolingual diameter of the crown was reduced to minimize the occlusal load and achieve optimal occlusal harmony. At the six-month follow-up,the fillings and the condition of the teeth were satisfactory, with no signs of periodontal complications. The patient's symptom of biting the cheek had disappeared, indicating a successful treatment outcome. This study has accumulated valuable clinical experience in the diagnosis and management of rare dental malformations. It indicates that modifying crown morphology can enhance the retention of multi-cusped molars with supernumerary cusps following root canal therapy, and highlights the importance of general dentists utilizing digital technologies in managing complex cases.
