Oral health-related quality of life and its influencing factors in elderly stroke patients under different functional states

Abstract:Objective To explore the oral health-related quality of life(OHRQoL)and its influencing factors in elderly stroke patients under different functional states,so as to provide a theoretical basis for formulating effective intervention measures.Methods A convenience sampling method was used to recruit 248 elderly stroke patients who met the inclusion and exclusion criteria from the outpatient clinics of Xuanwu Hospital,Temple of Heaven hospital,Daxing District Xihongmen hospital and Beijing stomatological hospital from October 2015 to January 2020.The subjects were initially screened and grouped based on the modified Rankin Scale score.There were 136 cases in the non-dysfunction group(0 or 1 point),112 cases in the dysfunction group[including 41 cases in the mild dysfunction group(2 points),36 cases in the moderate dysfunction group(3 points),and 35 cases in the severe dysfunction group(4 or 5 points)].The general information,oral care and treatment behavior information were collected through the questionnaire.The patients were assessed with oral health impact profile-14(OHIP-14),Montreal Cognitive Assessment(MoCA),Katz ability of daily living scale and Lawton instrumental ability of daily living scale for OHRQoL and general function,and their oral examinations were carried out.Objective to analyze the OHRQoL and its influencing factors of elderly stroke patients in different functional states by univariate analysis and multivariate analysis.Results There was no significant difference in the OHIP-14 scores between the non-dysfunction group and the dysfunction group(P>0.05);There was no significant difference in OHIP-14 scores among mild dysfunction group,moderate dysfunction group and severe dysfunction group(P>0.05).Univariate analysis showed that OHIP-14 total score was correlated with brushing frequency,MOCA score,number of remaining teeth and number of loose teeth(P<0.05).Multivariate analysis showed that the total score of OHIP-14 was correlated with the number of loose teeth,the number of remaining teeth,Katz score and whether there was a pension(P<0.05).Conclusion The OHRQoL of elderly stroke patients is closely related to oral health and functional status,so it is necessary to improve the oral health status of elderly stroke patients to improve their OHRQoL.
