Network Meta-analysis of the effects of different root canal filling pastes on the outcomes of root canal therapy

Abstract:Objective The influence of different root canal filling pastes on the outcome of root canal treatment was analyzed by means of network meta-analysis.Methods We searched CNKI,Wanfang Data Knowledge Service Platform,VIP,China Biomedical Literature Database,PubMed,Embase,Web of Science and Cochrane Library for randomized controlled trials of root canal filling sealant.The search time limit is 2011 to 2024,and the search is carried out by means of subject headings plus free words.Two reviewers independently screened and extracted data,assessed the risk of bias of the included studies according to the Cochrane Review Manual,used RevMan5.4 software,and used Stata 17MP and its network and mvMeta packages for statistical analysis based on the frequency framework.Results A total of 58 RCTs were included,involving 6 types of intervention measures(Bioceramics,epoxy resins,calcium hydroxide,silicone resins,zinc oxide clove oil,iodoform),with a total of 8374patients,4074in the experimental group and 4300 in the control group.Four outcome indicators were involved,among which 30 RCTs reported the effective rate,21 RCTs reported the success rate,9 RCTs reported the exact filling rate,and 8 RCTs reported the visual analogue scale(VAS)pain score 7 days after the operation.The network Meta-analysis showed that:The effective rate,success rate,filling rate and VAS pain score at 7 days after surgery,in the area under the cumulative ranking probability graph(SUCRA)were ranked first.Among the main outcome indicators,the top three primary intervention measures in terms of effective rate and success rate were bioceramic paste>epoxy resin paste>calcium hydroxide paste.Conclusion In terms of improving clinical effectiveness and success rates,bio-ceramic paste root canal filling materials have obvious advantages;in terms of proper filling rate and pain relief,bio-ceramic pastes perform the best.Due to the limitations of the number and quality of included studies,more high-quality clinical trials are still needed in the future for verification.
