Preventive effect of enhanced oral intervention combined with mechanical assisted expectoration on hypostatic pneumonia in elderly bedridden patients

Abstract:Objective To explore the preventive value of enhanced oral intervention combined with mechanical assisted expectoration on hypostatic pneumonia in elderly bedridden patients.Methods A total of 198 elderly bedridden patients admitted to Beijing Friendship Hospital Affiliated to Capital Medical University from March 2022 to March 2024 were selected and divided into a study group(99 cases)and a control group(99 cases)according to a random number table method.Based on the conventional intervention,the control group was given mechanical assistance for expectoration,while the study group was given enhanced oral intervention combined with mechanical assistance for expectoration.The improvement of chest X-ray after 2 weeks of intervention,the incidence of atelectasis pneumonia,the pulmonary function indicators before and after intervention,the respiratory difficulty score,the arterial blood gas indicators,the oral cleanliness and plaque index before and after intervention,and the intervention satisfaction were compared between the two groups.Results After intervention,the incidence of hypostatic pneumonia and dyspnea scores in the study group were lower than those in the control group(P<0.05,P<0.01);there was no statistically significant difference between the groups in the proportion of patients with hypostatic pneumonia showing ≥ 50%reduction in pulmonary shadow area after intervention(P=0.288).After 2 weeks of intervention,the maximal voluntary ventilation(MVV),forced vital capacity(FVC),forced expiratory volume in the first second(FEV1),partial pressure of oxygen(PaO2),and oxygen saturation(SaO2)in the study group were higher than those in the control group,while the partial pressure of carbon dioxide(PaCO2)in the study group was lower than that in the control group(P<0.01).The oral cleanliness in of the study group was better than that in the control group,and the plaque index after intervention was lower than that in the control group(P<0.01).The nursing intervention satisfaction in the study group was higher than that in the control group(95.96%vs 87.88%,P<0.05).Conclusion Enhanced oral intervention combined with mechanical assisted expectoration can improve oral cleanliness and hygiene status of elderly bedridden patients,alleviate dyspnea,improve pulmonary function and hypoxic state,effectively prevent the occurrence of hypostatic pneumonia,and achieve higher patient satisfaction with nursing intervention.
