Application of implant-supported short abutments mandibular complete over-denture to patients in poor outcomes with conventional complete dentures

Abstract：Objective To evaluate the application of implant-supported short abutments overdentures in patients with severe mandibular atrophy and poor outcomes from conventional complete dentures.Methods Between June 2014 and June 2017,46 patients with pronounced mandibular bone resorption and failed conventional denture rehabilitation at Dezhou Municipal Hospital underwent bilateral implant placement in the premolar region anterior to the mental foramen.Following osseointegration,overdentures were retained using 3-mm short abutments combined with pure titanium copings.After 6 months of restoration,the retention of the over-denture was evaluated and the changes in oral health life was also evaluated using the oral health impact profile(OHIP-14).CBCT and denture examinations were performed in the period of T0(the over-denture restored),T1(6th month),T2(1st year),T3(3rd year),T4(5th year)to assess bone resorption around implants and denture quality.Results The retention rate of the overdentures was significantly higher than that of traditional dentures(P＜0.01).The scores for oral function limitation,pain and discomfort,psychological discomfort,and physiological discomfort of the overdentures were lower than those of traditional dentures(P＜0.01).The bone resorption around the implants at different time points was T1(0.34±0.12 mm),T2(0.56±0.23 mm),T3(0.82±0.32 mm),and T4(1.17±0.34 mm),respectively.The resorption increased over time,and the difference was statistically significant compared with T0(P＜0.05).During the 5-year clinical observation period,one case of artificial tooth fracture due to a fall occurred,with an incidence rate of 2.17％.No implants were lost within 5 years,and the survival rate was 100％.Conclusion For patients with severe mandibular resorption,the use of short abutments combined with titanium crowns for retention can achieve fine effects,improve patient satisfaction,and is an economically effective method.
