Preliminary research on the fabrication and application of digital implants for jawbone defects combined with dentition defect

Abstract:Objective To observe the clinical application effect of digital personalized titanium stent implantation system with restorative abutment in patients with jaw defect combined with dentition defect. Method 48 patients with anterior jawbone defects and dental arch defects who were treated at the PLA 960th Hospital from January 2023 to June 2023 were selected. CBCT image data of patients with jawbone defects combined with tooth loss were obtained and imported into Mimics software to establish a three-dimensional digital jaw model and add personalized mesh implants. Personalized grid like implants were added and saved as STL format files for rapid prototyping,which were then input into a GIA type 3D printer. Processing parameters were set to drive rapid prototyping. Patients were divided into two groups,with personalized implants as the experimental group (24 cases) and finished titanium mesh combined with conventional implants as the control group (24 cases). Surgical time,postoperative redness and swelling time,postoperative pain time,implant stability test values and success rate data after 12 months of postoperative repair were obtained to compare the application effects of the two groups. Results a three-dimensional digital model of the jawbone defect site and surrounding tissues was successfully established,and a personalized mesh implant with a repair abutment was produced,with good surface integrity. The surgical time,postoperative redness and swelling time,and postoperative pain VAS score of the experimental group were all lower than those of the control group (P<0.01). After 12 months of follow-up after repair,3 patients in the experimental group had implant loosening,with a success rate of 87.5%. Two patients in the control group had implant loosening,with a success rate of 91.7%. The ISQ of the experimental group was lower than that of the control group (79.35±4.62 vs 86.46±6.74,P=0.000). Conclusion Through this method,personalized restoration of dental implants for mandibular and dental defects can be achieved,but its long-term application effects still need to be further studied.
