Clinical research on the influence of complete denture with two different kinds of occlusion for OHRQoL of elderly edentulous patients

Abstract:Objective:To investigate the influence between complete denture with long centric occlusion and anatomic occlusion Oral Health Related Quality of Life (OHRQoL) of elderly edentulous patients.Methods:According to random number table,20 elderly edentulous patients with severe residual alveolar ridge resorption with age of (71.1± 2.9) years old were randomly selected (11 males and 9 females) in the randomized cross-over controlled trial.For each patient,complete dentures with long centric occlusion and anatomic occlusion were fabricated and worn for 8 weeks in a randomized order respectively.All the patients were investigated with OHIP-EDENT (Oral Health Impact Profile,OHIP-EDENT) to evaluate the satisfaction degree of the patients.The Wilcoxon statistical test was used to compare the differences between two different occlusion.Results:Compared with anatomic occlusion,complete dentures with long centric occlusion were significantly superior in terms of "Dentures not fitting" (Z=-2.449,P=0.014<0.05),"Sore spots in mouth" (Z=-3.000,P=0.003<0.05) and "Interrupt meals" (Z=-2.236,P=0.025 <0.05).Conclusion:Long centric occlusion can contribute to decrease the incidence of sore spots and exert good masticatory function,which are conducive to reducing the influence of wearing complete dentures on taking food,being easier for aged edentulous patients with severe residual alveolar ridge resorption to accommodate and improving OHRQoL of patients.
