Autologous Incus for Ossicular Reconstruction in Type Ⅱ Tympanoplasty and its Postoperative Outcomes

Abstract:Objective To report outcomes of type Ⅱ tympanoplasty using autologous incus for ossicular reconstruction.Methods Data of 36 patients(36 ears)who underwent type Ⅱ tympanoplasty using autologous incus for ossicular reconstruction between January 2022 and November 2023 were retrospectively analyzed,including pre-and postoperative otoscopic examinations,and mean pure-tone hearing thresholds(500,1000,2000 and 4000 Hz)and air-bone gaps(ABGs)before surgery and at 3,6 and 12 months post-surgery.Outcomes assessment of hearing reconstruction included dry ear,ossicle extrusion and tympanic membrane perforation.Results Postoperative air conduction thresholds significantly improved compared to preoperative levels(58.99±11.73 dB HL at 3 months,57.26±11.13 dB HL at 6 months,and 57.25±11.29 dB HL at 12 months,compared to 66.36±11.90 dB HL,respectively).Bone conduction thresholds showed marginal improvement(40.15±12.27 dB HL at 3 months,40.26±12.15 dB HL at 6 months,and 40.36±12.18 dB HL at 12 months,compared to 41.76±12.26 dB HL,respectively).ABG progressively decreased(18.82±6.36 dB at 3 months,17.00±6.45 dB at 6 months,and 16.90±7.03 dB at 12 months,compared to 24.56±5.68 dB,respectively),ABG and was≤20 dB(considered successful reconstruction)in 77.8%of patients(28/36)at 3 months and 86.1%(31/36)at 6 months,which maintained unchanged at 86.1%(31/36)at 12 months.Delayed epithelialization was seen in 1 case(2.7%)at 3 months,with resolution by 6 months.No ossicle extrusion occurred.A single incidence of tympanic membrane perforation(2.7%)was observed.Conclusion The findings demonstrate that autologous incus can be effectively used for ossicular reconstruction in type Ⅱ tympanoplasty with satisfactory hearing outcomes across various postoperative intervals,as well as favorable biocompatibility and intraoperative malleability.In resource-limited healthcare settings and economically constrained patients,this technique provides a cost-effective alternative.
