Associations of Serum 25-Hydroxyvitamin D and Total Calcium Levels with Benign Paroxysmal Positional Vertigo:A Correlational Study

Abstract:Objective This propensity score-matched(PSM)retrospective cohort study aimed to investigate the association between serum 25-hydroxyvitamin D[25(OH)D]and total calcium levels and benign paroxysmal positional vertigo(BPPV).Methods A total of 107 BPPV patients diagnosed at the Department of Otolaryngology-Head and Neck Surgery,Beijing Tsinghua Changgung Hospital from June 2023 to January 2025 were enrolled as the case group,and 199 subjects from the Health Management Center served as controls.After 1:1 PSM matching for sex and age,74 matched pairs were included.Receiver operating characteristic(ROC)curve analysis was used to evaluate the predictive value of 25(OH)D for BPPV.Results Serum 25(OH)D levels were significantly lower in BPPV patients compared to controls[19.56(IQR 16.59,23.03)ng/mL vs 23.10(IQR 20.73,27.79)ng/mL,P=0.001],with a higher prevalence of vitamin D insufficiency(52.70%vs 14.86%,P<0.001).Female patients exhibited notably lower 25(OH)D levels than males[19.16(IQR 15.76,22.78)ng/mL vs 21.44(IQR 17.67,25.35)ng/mL].ROC analysis demonstrated that a 25(OH)D cutoff of<19.84 ng/mL predicted BPPV with 53%sensitivity and 87%specificity(AUC=0.700).Conclusion Low serum 25(OH)D levels are significantly associated with BPPV incidence,suggesting vitamin D deficiency may serve as a potential risk factor for BPPV.
