Clinical Outcomes of Endoscopic and Microscopic Tympanoplasty in Elderly Patients with Chronic Suppurative Otitis Media

Abstract:Objective To compare clinical outcomes of endoscopic and microscopical type Ⅰ tympanoplasty in elderly patients with chronic suppurative otitis media(CSOM).Methods Elderly patients with CSOM(n=90)who underwent Type Ⅰ tympanoplasty in the Department of Otolaryngology,the First Affiliated Hospital of Kunming Medical University,from April 2018 to December 2022 were retrospectively included and divided into two groups based on operation method:i.e.endoscopic via a tragonus-perichoniosteal approach or microscopic underlay repair using myofascia grafts.Hearing before surgery and the rate of successful healing,hearing improvement,quality of life and postoperative complications at 6 months after the operation were compared between the two groups.Results The rate of successful tympanic membrane healing in the endoscopic group was 95.00%,compared with 92.50%in the microscope group(P>0.05).Pre-operative and 6 months postoperative air and bone conduction thresholds,as well as air bone gap,showed no significant differences between the two groups(P>0.05),all showing improvement at 6 months compared to before surgery(P<0.05).Measurements in activity restriction,symptoms,medical resource consumption and total Chronic Ear Survey(CES)score showed no significant differences between the two group(P>0.05),with activity restriction,symptom,medical resource consumption and total CES score showing increased ratings at 6 months compared to before surgery(P<0.05).Conclusion Type Ⅰ tympanoplasty is safe and effective for elderly patients with CSOM,with similar efficacies with either endoscopic or microscopic techniques,provided appropriate and safe surgical methods are selected tailored to the individual patient's conditions.
