Influencing Factors and Mechanisms of Persistent Postural-Perceptual Dizziness Aggravated when Standing or Walking

Abstract:Persistent postural-perceptual dizziness(PPPD)is a chronic vestibular dysfunction disease,which is often aggravated by standing upright or walking and can seriously affect patients'quality of life.Age,sex,anxiety,neurotic personality and vestibular and non-vestibular comorbidities may affect exacerbation of dizziness in upright or walking state,and the mechanism may be related to changes in cerebral cortical network connectivity and activity,as well as changes in postural control strategies,visual and somatosensory dependence,and neurotransmitter abnormalities.This article provides a review on the influencing factors and possible mechanisms of aggravated symptoms in PPPD when standing or walking,in order to gain a deeper understanding of this pathophysiological phenomenon and provide a basis for improving its therapeutic outcomes.
