Causal Association of Micronutrients with Risk for Benign Paroxysmal Positional Vertigo of Multivariate Mendelian Randomization Study on European Population

Abstract:Objective To investigate the relations between levels of 15 micronutrients and the risk for benign paroxysmal positional vertigo(BPPV)using Mendelian randomization(MR).Methods Multivariate MR analysis was performed using the genome-wide association study(GWAS)database to obtain data on the levels of 15 micronutrients(copper,calcium,carotenoids,folate,iron,magnesium,potassium,selenium,zinc,vitamin A,vitamin B12,vitamin B6,vitamin C,vitamin D,and vitamin E)and related BPPV Data.With the R software and TwoSampleMR package,the causal relationship between micronutrients and BPPV was analyze using inverse variance weighting(IVW)as the main method,supplemented by MR-Egger regression,weighted median,simple model and weighted model methods.Heterogeneity and horizontal pleiotropy of single nucleotide polymorphism(SNP)were detected by the Cochran Q test,MR-Egger regression test,MR-PRESSO composite test,and MR-Egger intercept to enhance results reliability and stability.Multivariate MR analysis was performed on the relevant trace elements screened by MR to avoid interference between variables,analyze the independent effects of trace elements,and further improve reliability of the study.Results The prevalence of BPPV was negatively associated with Folic acid(IVW:OR=0.721,95%CI:0.527-0.986,P<0.05),but positively associated with calcium(IVW:OR=1.472,95%CI:1.016-2.133,P<0.05)and selenium(IVW:OR=1.074,95%CI:1.005 to 1.148,P<0.05).Elevated serum zinc increased the risk for BPPV(IVW:OR=1.077,95%CI:1.005 to 1.155,P<0.05).Folic acid and calcium were independent effects on the risk for BPPV(P<0.001).There was no heterogeneity or horizontal pleiotropy among the folate,selenium and zinc instrumental variables.Leave-one-out test analysis confirmed that individual SNPs had no significant effect on the overall results.Conclusion Folic acid,calcium,selenium and zinc are causally associated with the development of BPPV,with folic acid as a protective factor,and calcium,selenium and zinc as risk factors,which may provide a basis for understanding BPPV pathogenesis,as well as its prevention and early intervention.
