Optimization of Diagnosis and Treatment for Vertigo Diseases and Application in Primary Care Hospitals

Abstract:Objective To report authors’experiences in establishing an integrated multidisciplinary treatment(MDT)team led by otolaryngologists in a county level hospital to optimize and promote an easy to implement diagnostic and treatment model for common dizziness disorders aimed to enhance capabilities and health economic benefits at primary care level medical centers.Methods The study included 463 dizziness patients treated following a optimized diagnosis and treatment model at Yunyang County People's Hospital in Chongqing from March 2022 to August 2022(study group),and 415 dizziness patients treated from March 2021 to August 2021(control group).Hospitalization rates,diagnostic and treatment costs,average length of hospital stay and satisfaction rates were compared between the two groups.Additionally,diagnosis of dizziness and Benign Paroxysmal Positional Vertigo(BPPV),and outcomes of repositioning for BPPV at three central community health centers using the same diagnosis and treatment model were reported.Results There was no significant difference in the average length of hospital stay or treatment costs between the two control groups(P>0.05),although hospitalization rate was lower in the study group(P<0.05)with lower treatment costs(P<0.05).For BPPV,there was no significant difference in inpatient treatment costs between groups(P>0.05),but the hospitalization rate,outpatient treatment costs and overall treatment costs were lower in the study group(P<0.05),with better control of dizziness(98.92%,458/463)than in the control group(96.14%,399/415)(χ2=6.081,P=0.014<0.05).The rate of patient satisfaction in the study group(95.25%,441/463)was also higher than in the control group(91.81%,381/415)(χ2=4.340,P=0.037<0.05).With the model implemented,diagnosis of dizziness at the three central community health centers decreased(P<0.05),while diagnosis of BPPV increased,with no change in the success rate of BPPV reduction(P>0.05).Conclusion The optimized diagnosis and treatment model enhanced treatment capabilities for dizziness and is suitable for application in primary care settings,offering considerable health economic benefits.This model has the potential to improve dizziness diagnostic and treatment in county-level hospitals.
