A Study on the Correlation between Age-related Hearing Loss and Sleep,Emotion and Cognitive Function

Abstract:Objective To determine the correlation between age-related hearing loss(ARHL)and sleep,emotion and cognitive function in the elderly.Methods Data from 147 ARHL patients aged 60 and above(ARHL group)and 33 elderly individuals with normal hearing(control group)admitted to the Second Affiliated Hospital of Xi'an Jiaotong University from November 2020 to November 2022 were retrospectively analyzed.Unilateral tinnitus was reported in 50 cases,bilateral in 66 cases and none in 64 cases.In the ARHL group,hearing loss was mild in 47 cases,moderate in 62 cases,and severe or worse in 38 cases,based on the pure tone average(PTA)results of the better ear.The Pittsburgh Sleep Quality Index(PSQI),Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS)and Montreal Cognitive Assessment(MoCA)were used to evaluate sleep,emotion and cognitive function.Results There was no statistically significant difference(P>0.05)in PSQI scores,sleep quality,sleep duration,sleep disturbance,use of sleep medications,and daytime dysfunction among patients with or without tinnitus,although there were statistically significant differences(P<0.05)in sleep latency and sleep efficiency.SAS and SDS scores showed statistically significant differences among patients with different hearing loss levels(P<0.05),and higher in patients with mild hearing loss group were than in the control group(P<0.05).There were statistically significant differences(P<0.05)in total MoCA score,visual space and execution,nomination,attention,language,abstraction,delayed recall,and orientation among the control group and patients with various levels of hearing loss.The MoCA score was negatively correlated with PTA,gender,and labor intensity(P<0.001).Conclusion As patients with age-related hearing loss age,their hearing loss worsens.Without timely intervention,they can develop anxiety and depression,cognitive impairment and tinnitus,which can affect sleep quality.In the early stage of the disease,hearing loss may especially have a greater impact on the physical and mental wellbeing of the elderly.
