Abstract:Objective To report clinical characteristics,diagnostic methods and treatment strategies of antineutrophil cytoplasmic antibody-associated vasculitis(AAV)initially diagnosed as otitis media.Methods Data from 3 patients with AAV initially diagnosed as otitis media who were jointly treated by the Department of Otolaryngology-Head and Neck Surgery and the Department of Rheumatology and Immunology,the Second Hospital of Shanxi Medical University,were retrospectively analyzed,including clinical characteristics,diagnostic criteria and therapeutic outcomes.Results The patients initially presented with recurrent tympanic effusion and various types of hearing loss.Temporal bone CT scans suggested otitis media and mastoiditis.Otoscopy showed tympanic membrane congestion and thickening,as well as tympanic effusion.Conventional anti-infection,eustachian tube function management and tympanic membrane puncture were ineffective.ANCA series tests were positive.After immunosuppressant combined with glucocorticoid treatment in the Department of Rheumatology and Immunology,symptoms resolved in 2 cases relieved and the other patient died.Conclusion Otorhinolaryngologists need to improve their understanding of the disease.If"otitis media"does not respond to conventional treatment,immunology tests should be considered to diagnose ANCA-associated vasculitis for early and targeted treatment to control disease progression and improve prognosis.
