Correlation between Serum Otolin-1 Level and Occurrence of Residual Symptoms after Successful Treatment of Benign Paroxysmal Positional Vertigo

Abstract:Objective To investigate the correlation between the serum level of inner ear specific protein Otolin-1 and the occurrence of residual dizziness(RD)after successful treatment of benign paroxysmal positional vertigo(BP-PV).Methods Patients who were diagnosed with new onset idiopathic BPPV at Ningbo No.2 Hospital,from May 2017 to May 2018 were recruited.All patients underwent canalith repositioning procedures and were grouped based on the presence of RD during followed ups.Demographic data and comorbidities were collected.Peripheral venous blood was drawn for measurement of Otolin-1 and other laboratory-related indicators.Results A total of 69 patients with BP-PV were enrolled,with 40.6%(28/69)of them experienced RD.Serum level of Otolin-1 was 357.4(313.6,411.4)pg/mL in the RD group and 312.8(265.3,345.7)pg/mL in the non-RD group,respectively,(Z=-3.348,P=0.001).The receiver operating characteristic(ROC)curve showed a cut-off value at 330.81 pg/mL and the area under the curve for predict-ing the occurrence of RD after successful treatment in patients with BPPV was 0.739(95% CI:0.622,0.856).Using the cutoff value,subjects were divided into an elevated Otolin-1 group and a normal Otolin-1 group.Logistic regression analysis showed an increased risk for RD associated with elevated Otolin-1,with an odds ratio of 10.06(95% CI:2.33,43.42,P=0.002).Conclusion Serum Otolin-1 levels were significantly elevated in patients who experienced RD after canalith repositioning treatment for BPPV,suggesting that serum Otolin-1 levels may be used as a serological marker to predict RD after successful treatment in BPPV.
