Three Cases of Mastication Induced Vertigo and Literature Review

Abstract:Objective To report clinical characteristics and treatment outcomes in 3 patients with vertigo induced by mastication.Methods Characteristics of three patients with mastication induced vertigo were recorded,including ves-tibular function and pure tone audiometry test results.Results All three patients were male,with an average age of 47.7 years.Mastication-induced vertigo lasted from several to tens of seconds.The vertigo was accompanied by left horizon-tal nystagmus or right horizontal plus counterclockwise rotary nystagmus.One patient was found to have bilateral high-frequency(4 and 8 kHz)hearing loss on pure tone audiometry,whereas hearing was normal for the other two pa-tients.There was no spontaneous nystagmus,although two patients showed nystagmus after head shaking.Smooth pur-suit and voluntary fixation suppression were normal in all three patients.Unilateral low or absent oVEMPs were seen in two patients,while cVEMPs were within normal limits in all patients.MRI in all patients suggested multiple foci of ischemia in bilateral ventricle and basal ganglia areas.Conclusions The three patients were determined to have peripher-al vestibular vertigo,with characteristics mimicking paroxysmal vestibular disorders.All three patients were successful-ly treated with carbamazepine.
