Diagnosis and Treatment of Otogenic Thrombophlebitis of Sigmoid Sinus(A Report of 6 Cases)

Abstract:Objective To report the etiology, clinical presentation and to therapeutic strategies for thrombophlebitis of sigmoid sinus in a small case series. Methods Cases of thrombophlebitis of sigmoid sinus seen from May 2008 to May 2017 at the Chinese PLA General Hospital were reviewed of clinical presentations, lab results, intraoperative find-ings, surgical approaches and postoperative follow-ups. Results Sigmoid sinus thrombophlebitis was diagnosed through imaging before operation in 3 cases, while diagnosis could not be confirmed until the operation in the other 3 cases. Cho-lesteatoma was diagnosed in all 6 cases and successfully eradicated with no recurrence during the follow up from 3 months to 9 years after operation. Conclusions History and imaging are important for the diagnosis of thrombophlebitis of sigmoid sinus. Surgical treatment combined with antibiotics is the most direct and effective management for thrombo-phlebitis of sigmoid sinus. In adult patients with cholesteatoma, relevant symptomatology should alert the clinician of the possibility of sigmoid sinus thrombosis.
