CO2 and Er:YAG laser mastoidotomy and its effects on surrounding middle ear bone tissue

Abstract:Objective To study the effects of CO2 and Er:YAG laser bone ablation on surrounding bone tissue using a New Zealand Rabbit middle ear model to determine the feasibility and safety of laser mastoidotomy in ear surgery. Methods Twenty-four New Zealand Rabbits were randomly divided into three groups to undergo in-vivo mastoid bone ablation by the means of drilling, CO2 laser or Er:YAG laser. Specimens of the ablation crater were examined by scanning electron mi-croscopy. HE staining was used to assess thermal damage in the surrounding bone tissue. Results Collagen fusion and cracks were seen on scanning electron microscopy on the surface of the crater bone. Laser bone ablation caused more dam-age to surrounding bone tissue than bone cutting by drilling (P<0.01), but there was no significant difference between the two laser groups (P>0.05) or between different sampling locations (P>0.05). Conclusions The two laser systems could ab-late the bone rapidly in mastoidotomy operations, although they can cause more damage to surrounding bone tissue than tra-ditional drilling. Further study is needed for optimization of laser techniques for large bone ablation.
