Combined intratympanic lidocaine with masking therapy and psychological intervention in treating tinnitus

Abstract:Objective To report effects of intratympanic lidocaine injection combined with masking and psychologi-cal intervention for idiopathic tinnitus. Methods Patients (n=177) meeting inclusion criteria were selected to receive intra-tympanic lidocaine injection with masking (lidocaine group, n=59), psychological intervention with masking (psychological intervention group, n=59) or all three therapies (combination group, n=59). Treatment efficacy was evaluated by tinnitus se-verity assessment and scoring, as well as SCL-90 scoring, before and at 1 and 3 months following treatment. Results 1) At 1 month and 3 months following treatment, there were differences among the groups in treatment efficacy (Hc=7.94, P=0.019 and Hc=7.01, P=0.03, respectively), with efficacy in the combination group being superior over the other two groups. 2) Af-ter treatment, SCL-90 scores indicated changes regarding somatization, interpersonal sensitivity, depression, anxiety, hostili-ty and paranoid factor in the psychological intervention and combination groups (P<0.05), but not in the lidocaine group. Conclusion Intratympanic lidocaine injection combined with masking and psychological intervention appears to produce therapeutic effects superior over intratympanic lidocaine injection with masking or psychological intervention with masking. While therapeutic effects were maintained in all three treatment combinations, psychological intervention seemed to be help-ful in patients with tinnitus.
