Light cupula:A new theory in explanation of persistent geotropic direction-changing positional nystagmus

Abstract:In recent years, a unique direction-changing positional nystagmus (DCPN), with no latency or fatigability but a null point when head is turned toward the affected side, has been described. Different from the transient geotropic DCPN seen in BPPV from horizontal semicircular canal, it is just the reverse of the pathogenic mechanism of persistent apo-geotropic DCPN. Light cupulae theory has been used to explain the characteristics of this unique nystagmus. In this paper, the light cupulae, characteristics of DCPN, possible etiologies, pathogenesis hypotheses, treatment and prognosis, and the correlation between DCPN and BPPV are reviewed.
