Positive-negative pressure balance combined with Valsalva’s maneuver in evaluating eustachian tube function in patients with chronic otitis media compared to those with tympanic membrane perforation

Abstract:Objectives To assess eustachian tube function and the incidence of eustachian tube dysfunction in patients with chronic otitis media and in those with tympanic membrane perforation. Methods Positive-negative pressure balance was used in combination with Valsalva’s maneuver to assess eustachian tube function in 42 patients (46 ears) with traumatic tympanic membrane perforation and 225 patients (248 ears) with chronic otitis media. Results Voluntary and passive eusta-chian tube opening in patients with traumatic tympanic membrane perforation were superior over those with chronic otitis media. The rate of eustachian tube dysfunction was 2.17%(1/46) in the former group and 76.21%(189/248) in the latter. Conclusions In patients with chronic otitis media, eustachian tube opening under negative pressure is the first to be affect-ed, with an increased incidence of eustachian tube dysfunction, mainly due to poor tensor veli palate functions that lead to impaired voluntary tube opening.
