Autogenous ossicles in ossiculoplasty

Abstract:Objective To report outcomes of ossiculoplasty with autologous ossicles. Methods Fol-low-up data of 58 patients (60 ears) who underwent ossiculoplasty using autologous auditory ossicles were an-alyzed retrospectively. Patients were followed up for 3-18 months with an average of 11.6 months. Pre-and post-operative pure tone average (PTA) air-conduction thresholds, bone-conduction thresholds and air-bone gap (ABG) at 0.5, 1 and 2 kHz were analyzed. Results There were no postoperative autologous ossicle grafts extrusion. There were graft displacement in three cases without tympanic membrane perforation or invagina-tion. All patients achieved dry ear postoperatively. Air conduction thresholds improved from a preoperative av-erage of 54.7±6.4 dB HL to a postoperative average of 39.0±9.1 dB HL, while bone conduction thresholds im-proved from a preoperative average of 28.3 ± 4.6 dB HL to a postoperative average of 22.2 ± 4.2 dB HL, with air-bone gaps decreased from a preoperative average of 26.4 ± 7.9 dB to a postoperative average of 16.8 ± 8.2 dB. Postoperative PTA-ABG was reduced to 20 dB or less in 71.7%(43/60) of the cases. Conclusion Consid-ering the convenient availability, low expense, high levels of histocompatibility, low rate of complications and excellent hearing results, autogenous ossicles, with minimal reshaping, are ideal materials that can be success-fully applied in ossiculoplasty.
