A comparative analysis of hearing changes in tympanosclerosis and middle ear cholesteatoma before and after surgical treatment

Abstract:Objective To study hearing outcomes following surgical treatment in tympanosclerosis and middle ear cho-lesteatoma. Methods Included in the study were 36 cases of middle ear cholesteatoma and 31 cases of tympanosclerosis. The air-bone gaps (ABG) at 500, 1000, 2000 and 4000 Hz before and after operation were compared. Results The serum cal-cium level was not significantly different between the two diagnoses groups. Preoperative hearing loss in tympanosclerosis was worse than in middle ear cholesteatoma. Postoperative hearing outcomes showed significant improvement in both diag-noses groups with no statistically significant difference between the two groups. Both groups showed significant reduction of air-bone gaps at 500 Hz and 1000 Hz after operation, but not at 4000 Hz. Postoperative hearing in patients with middle ear cholesteatoma was significantly better than in patients with tympanosclerosis at 2000 and 4000 Hz. Conclusions Hear-ing outcomes following surgical treatment are similar in tympanosclerosis and middle ear cholesteatoma. Postoperative hear-ing outcomes are especially better at mid and low frequencies.
