Videonystagmography (VNG) Characteristics in Vertigo Sufferers with Congenital Nystagmus:A Report of 2 Cases

Abstract:Objectives To assess the diagnosis value of VNG in vertigo sufferers with congenital nystagmus. Methods Sensory defect nystagmus sufferers and motion defect nystagmus sufferers received VNG tests, including spontaneous nystag?mus, static positional nystagmus, positioning nystagmus, saccade random test, pursuing test, caloric test and fixation test. Re?sults For congenital nystagmus sufferers with constant direction and slow phase velocity (SPV), their saccade random and pur?suit curves followed well with the curve of target, although superimposed with constant and fine congenital nystagmus waves. Undershot was detected in pursuing test toward the side of low vision and typeⅢcurve was found in 0.4 Hz pursuit test. Calor?ic and fixation tests were not affected in patients with constant horizontal congenital nystagmus. The VNG results in position?ing test for diagnosing BPPV were too disordered to be analyzed for recognizing disturbance of congenital nystagmus. Conclu?tions Congenital nystagmus is not an absolute contraindication to VNG test, Saccade random test, pursuit test, caloric and fixa?tion test are useful and valuable in vertigo localization diagnosis.
