Abstract:Malignant pleural effusion is a common complication of advanced lung cancer, which significantly reduces patients'quality of life, shortens overall survival, and is associated with a poor prognosis. The theory of"Xuan Fu,Qi,and body fluids"was developed by Liu Wansu based on the Plain Questions, emphasizing that Xuan Fu should remain unobstructed as pathways for the movement of Qi and body fluids.Based on this theory, this article proposes that the occurrence of lung cancer complicated with malignant pleural effusion is initiated by the obstruction of Xuan Fu,with the stagnation of Qi and body fluids as the core pathogenesis. The disease course can be divided into acute, stable, and recovery stages. The acute stage focuses on symptom relief, employing treatments to warm Yang, activate blood circulation, promote diuresis, and open Xuan Fu.The stable stage aims to slow disease progression and guide recovery, using methods to strengthen the spleen to control water retention, remove stasis, and unblock Xuan Fu,thereby achieving the goal of"preventing deterioration after disease onset".The recovery stage prioritizes treating the primary disease to eliminate its root cause, employing strategies to consolidate lung function, regulate Xuan Fu,and stabilize the body's foundation, thereby accomplishing the objective of"preventing recurrence after recovery".Clinical case analyses are also provided to offer reference for syndrome differentiation and treatment in clinical practice.
