Exploration of Clinical Approaches to the Diagnosis and Treatment of Malignant Pleural Effusion Using Jiao Medicine

Abstract:Malignant pleural effusion(MPE),a common complication in advanced malignancies,is characterized by high incidence and mortality rates,significantly compromising patients'survival duration and quality of life.Traditional Chinese medicine(TCM)demonstrates distinct advantages in inhibiting and delaying MPE progression,alleviating clinical symptoms,and improving patients'quality of life.In TCM theory,MPE is classified as"suspended fluid retention"(Xuan Yin).Its pathogenesis originates from deficiency of vital Qi as the constitutional basis,with cancer toxin persisting throughout the disease course.The dysregulation of visceral Qi dynamics leads to accumulation of pathological products including pathogenic fluids,phlegm-dampness,and blood stasis,which obstruct the triple energizer waterways and ultimately result in disordered fluid metabolism with localized effusion.In clinical practice,the author adopts a therapeutic strategy of promoting righteousness and combating cancer while regulating Qi and facilitating fluid excretion.This holistic approach considers different primary tumors and the various stages of disease progression,integrating syndrome differentiation,disease differentiation,and constitution differentiation to flexibly apply Jiao medicine,often achieving favorable results.This article summarizes eight groups of Jiao medicine and includes one case report,aiming to provide references and insights for the clinical application of Jiao medicine in the diagnosis and treatment of MPE.
