Study on the Mechanism of Shenqinlong Qingfei Peiyuan Granules on Autophagy of Peripheral Blood Macrophages in HIV/AIDS Patients with Pulmonary Infection and Phlegm-Heat Obstructing the Lung Syndrome

Abstract：Objective To investigate the mechanism of action of Shenqinlong Qingfei Peiyuan granules based on macrophage autophagy.Methods A total of 36 patients with human immunodeficiency virus(HIV)/acquired immunodeficiency syndrome(AIDS)complicated by pulmonary infection with phlegm-heat obstructing the lungs syndrome were selected.Flow cytometry was used to assess the peripheral blood T lymphocyte subpopulations,including CD4+and CD8+T lymphocyte counts,before and after treatment.The expression of macrophage subpopulations(CD11b,CD86,CD206)and the expression of macrophage autophagy probes(CD11b+DALGreen,CD86+DALGreen,CD206+DALGreen)were also analyzed.Enzyme-linked immunosorbent assay(ELISA)was employed to measure the levels of inflammatory cytokines,including tumor necrosis factor(TNF)-α,interleukin(IL)-1β,IL-10,and transforming growth factor(TGF)-β1,before and after treatment.Quantitative PCR was used to detect the expression of HDAC2 mRNA and ULK1 mRNA in peripheral blood before and after treatment.Results After treatment,the expression of CD11b,CD206,CD11b+DALGreen,CD206+DALGreen,and ULK1 mRNA significantly increased(P<0.05).Conversely,the expression of CD86,TNF-α,IL-1β,and HDAC2 mRNA significantly decreased(P<0.05).The CD4+T lymphocyte count,as well as the expression levels of IL-10 and TGF-β1,showed an increasing trend(P>0.05).Conclusion Shenqinlong Qingfei Peiyuan granules may exert their therapeutic effects in HIV/AIDS patients with pulmonary infections by regulating the HDAC2/ULK1 signaling axis.This regulation promotes macrophage autophagy,modulates the M1/M2 macrophage polarization balance,and reduces the inflammatory response,thereby improving clinical outcomes.
