Exploration of the Pathogenesis,Differentiation and Treatment of IgA Nephropathy Under the Guidance of the Theory of"Ying Wei Qing Yi"Based on Mucosal Immune Mechanism

Abstract:Modern medical research indicates that mucosal immune responses and infections are closely associated with the onset and progression of IgA nephropathy(IgAN).Traditional Chinese medicine(TCM)primarily employs theories such as Wei-Qi-Ying-Xue syndrome differentiation,Sanjiao syndrome differentiation,Latent pathogens in pathogenesis and Zang-Fu organ correlation to explain IgAN.However,these theories cannot fully and accurately encompass the entire disease course and outcomes.Based on the functional and anatomical similarities between the TCM concept of"Wei-Qi"and the mucosal immune system,this study proposes exploring the pathogenesis and treatment of IgAN through the theory of"Ying Wei Qing Yi".The core pathogenesis are exogenous or internal injuries causing disharmony between Ying and Wei,"Ying Wei Qing Yi"leading to obstruction of the renal collaterals,and weakness of Ying-Wei along with lingering pathogenic factors.Corresponding therapeutic strategies and prescriptions are proposed,including:expelling pathogens and reinforcing defensive Qi,harmonizing Ying and Wei;clearing heat and cooling blood,unblocking Ying-Wei circulation;and consolidating the root and eliminating pathogens,nourishing Ying-Wei.This approach provides novel insights and methodologies for the systematic diagnosis and treatment of IgAN.
