Abstract:The theory of blood turbidity holds significant importance in the diagnosis and treatment of chronic kidney disease(CKD).Given the vital role of the kidneys in maintaining the normal circulation and purity of blood,a deficiency in kidney Qi leads to the generation of turbidity within the blood. This paper delves into the concept, fundamental nature, and pathogenic characteristics of blood turbidity, while also examining its relationship with CKD during the pathogenesis. We contend that the core pathogenesis of CKD revolves around kidney Qi deficiency and the subsequent production of blood turbidity by the kidneys.Blood turbidity, traveling with the blood, exhibits varied manifestations.Consequently, the combination of turbidity and dampness, the intermingling of turbidity and phlegm, the formation of turbidity-induced blood stasis, and the transformation of turbidity into toxicity are pivotal factors in the progression of CKD.An excess of blood turbidity that harms the five-viscera marks the ultimate outcome of CKD.Therefore,we propose reinforcing kidney Qi and clearing blood turbidity as the core therapeutic approaches for CKD.Simultaneously, incorporating dampness-resolving and phlegm-eliminating herbs,blood-stasis-dispersing medications, and detoxifying agents to address related pathological factors, supplemented by blood-regulating and Qi-circulating therapies to ensure smooth Qi and blood circulation,is essential. In the end-stage of CKD,when all five-viscera are impaired,we advocate for the simultaneous regulation of the Zang-Fu viscera and the prevention of disease progression to halt the advancement of CKD,aiming to provide a syndrome differentiation-based approach for clinical practice.
