Clinical Characteristics,Combination Therapy Patterns,Rational Analysis of Shengmai Injection in the Treatment of Malignant Tumors in the Real World

Abstract：Objective Based on the real world data,to explore the population and drug characteristics of Shengmai injection in the treatment of malignant tumors,to provide reference for clinical use.Methods The hospital information system (HIS) of 7493 patients with malignant tumor treated with Shengmai injection was extracted from 29 Grade-A hospitals in China from 2002 to 2019.Excel and SPSS Clementine software were used for statistical processing.Results Among the 7493 patients treated with Shengmai injection for malignant tumor,the male was more than the female,the ratio was about 1.8︰1.The majority were aged from 66 to 85,accounting for 42.65％;Agricultural and forestry industries accounted for 16.94％ of the total.The most patients were admitted in spring,accounting for 28.28％.Tumor diagnosis and treatment center accounted for 23.21％.Most patients were admitted through outpatient service and medical insurance.Most of the patients admitted to hospital were general,accounting for 69.77％.The average length of hospital stay was 26.25 days.The average hospitalization cost was 31144.88 yuan.The top 5 in Western medicine diagnosis were lung malignant tumor,liver malignant tumor,stomach malignant tumor,bone secondary malignant tumor and liver secondary malignant tumor.The top 5 TCM syndrome diagnosis were phlegm-stasis syndrome,Qi and Yin deficiency syndrome,Qi and blood stasis syndrome,Qi and blood stasis syndrome,Qi and blood deficiency syndrome,respectively.The analysis of combined drug use showed that dexamethasone was the most common combination of western drugs,accounting for 6.61％.The most common combination of TCM was Bupleurum injection,accounting for 6.82％.The analysis of drug combinations showed that antimicrobial drugs were most commonly used with western medicine system,accounting for 9.44％.It was most commonly used in combination with Chinese herbal heat clearing agent,accounting for 19.08％.Association rule analysis showed that spironolactone combined with furosemide was the most common combination.The most common combination of traditional Chinese medicine and Western medicine was compound fresh bamboo leach solution,Tanreqing injection combined with ambroxol.The main administration route of Shengmai injection was intravenous drip,accounting for 85.23％.The average single dose was 57.09 mL.The average treatment course was 4.94 days.2076 patients (27.71％) were better when discharged from hospital.Conclusion In the real world,Shengmai injection is used in the treatment of malignant tumors with distinct characteristics,and the combination of drug use is reasonable,but some Shengmai injection is not standardized.It is suggested that clinicians strictly abide by the indications,usage and dosage,medication contraindications and precautions stipulated in the instructions,so as to enhance the theoretical study of traditional Chinese medicine and rational differentiation of drug use,and improve clinical efficacy and ensure drug safety.
