A case of Talaromyces marneffei infection combined with herpes zoster in an immunodeficient patient with anti-IFN-γ autoantibodies

Abstract:A 48-year-old female patient with anti-IFN-γ autoantibody positive immune deficiency disease was reported.The patient presented with a 4-month history of cervical lymphadenopathy followed by progressive disseminated cutaneous papulonodular eruptions and recurrent fevers persisting for more than a month.After admission,the patient quickly developed multiple organ dysfunction syndrome,metagenomic next-generation sequencing(mNGS)was used.Talaromyces marneffei was detected in the puncture fluid of the left leg mass and the arm abscess,while histopathological examination of dorsal lesions demonstrated characteristic fungal elements.Concurrent laboratory investigations showed elevated(1,3)-β-D-glucan levels(G-test positivity).After diagnosis,amphotericin B and voriconazole were given for a full course of antifungal therapy.Herpes zoster occurred during treatment,and the patient improved with antiviral therapy and revised antifungal therapy.After a total of 12 weeks of systemic antifungal therapy,the patient achieved clinical cure during outpatient follow-up.
